The answer is the Vampirella/Shi Limited Ash Can. One of the few ash cans that I own. And you're right the pen- cil sketch in the inside back cover is sweet!

QUITE right on all counts. Here are the links I had: Typhoid Mary appears in Daredevil comics. Daredevil crossed over with SHI.
Purgatori was in Lady Death issues. Lady Death and SHI appeared in Wizard Bad Girls Special.
